Nudimo dugoročnu pomoć u obavljanju zadataka poput kupovine, vrtlarstva, čišćenja i pranja veša. Empowering Independence: Dana’s Story
We believe that everyone deserves to lead a secure and independent life, especially those participating in the Commonwealth Home Support Programme (CHSP) and Home Care Packages (HCP). By understanding and supporting each individual’s unique needs, we help build capacity and improve their independence.
CHSP is designed for people with low level needs and provides a few basic levels of care.
HCPs are split into four different tier levels depending on the individual’s needs.
Take Dana, for example. At 80 years old, Dana lives alone and found herself needing additional help with house cleaning, shopping, and garden maintenance. After being assessed and receiving the level 2 HCP package, her St Simeon care coordinator, Gina, stepped in to match Dana with the right home carer who could provide the support she needed while maintaining her independence at home.
